1. Participants must be 18 years or older to qualify and/or win.
2. Winners must provide name, address, phone number, date of birth, and social security number.
3. All prizes are nonrefundable and non-redeemable.
4. No cash substitution will be available.
5. Prizes may be substituted should they be unavailable due to circumstances beyond our control.
6. Prizes may not be sold nor be used in direct promotion of any business.
7. Winner shall be solely responsible for all applicable taxes, fees and duties, including any federal, state or local taxes.
8. No employees of Healing Lifesyles & Spas, JLD Publications, Indiana Gazette Printers, or any organizations or agencies affiliated with them and their immediate families shall be eligible to participate.
9. Winners shall, if requested, execute a liability release. Failure to sign a release shall result in forfeiture of the prize.
10. Due to unforeseen circumstances, the contest can be discontinued at any time during the promotional period.
11. There shall be only one prize winner per household per contest.
12. If for any reason, any contest is not capable of being run as planned, due to infection by computer virus, bugs, tampering, malicious files or viruses or other causes beyond our control, we reserve the right, in our sole discretion, to cancel, terminate, modify, or suspend the contest.
13. Participating contest winners agree to have their name used in any advertising relating to the contest and must sign a waiver stating so prior to receiving the prize.
14. Failure to comply with contest rules may result in disqualification.
15. No more than one prize or gift may be awarded to a household in any 30-day period.
16. We do not make any warranties regarding the safety, condition, or fitness of any prize awarded, and shall not be responsible for any loss, damage, or injury suffered by the winner through the use of any awarded prize.
17. Winners of any contest must claim the prize within thirty (30) days of notification or the prize will be forfeited.
18. We are not responsible for mail that has been lost, damaged, or delayed.
19. Winners of prizes consisting of trips or spa packages must book such trips at least sixty (60) days in advance, and may not be scheduled during holiday periods without permission of the owner of the facility.
20. No material or any portion of this website may be copied, duplicated, or reproduced without our written consent.
21. If requested, the participant must fill out the survey to receive free gift or to be eligible for a contest.
22. Participants must be active subscribers for one month in order to receive contest prize for Daily Retreat & Renew Tip Program.
